WestCoast Children’s Clinic, located in Oakland, California, is a non-profit community psychology clinic that provides mental health services to Bay Area children, youth and families. The Bilingual (Spanish) Care Coordination Specialist (Intensive Care Coordinator) is responsible for engaging and coordinating care for youth/young adults and their family members, foster parents, or other support persons. This position identifies and brings together family, natural supports (friends, extended family, neighbors, community members) and formal system partners (child welfare, school, juvenile probation, etc.) to form a Child & Family Team (CFT) which collectively works together to address goals and ensure successful and enduring change.Â
The Bilingual Care Coordination Specialist coordinates and facilitates regular CFT meetings among all the teams on their caseload, and they monitor and contribute to progress toward the youth’s goals. This position also provide a diversity of care through conducting short term needs and strengths assessments and providing stabilization and comprehensive support services to foster children and TAY in a 24 hr drop-in center. An ideal candidate is outgoing and demonstrates flexibility, initiative, creativity, inclusivity and knowledge of the unique challenges youth and young adults face in foster care.
Bilingual Care Coordination Specialists provide services and participate in agency meetings in a combination of environments including virtual (zoom / phone) and in-person office or community locations (drop-in center, client homes, school, parks, etc.)
How you'll make an impact:
- Outreach to and engagement with youth; Being a strong advocate for the youth within the team
- Provide community-based trauma-informed services in Spanish to foster youth or former foster youth transitioning to or struggling with independent living.Â
- Provide intensive skill building with youth, with a focus on independent living.
- Provide assessments, crisis intervention, stabilization, and safety plans.Â
- Identify and connect youth & young adults with other agencies to address their needs, including housing, medical, mental health, education, employment, advocacy, placement stability, and enrichment activities.Â
- Work collaboratively in partnership with the team to engage and connect clients to appropriate resources and services.
- Assist with initial contact and intake, establishing relationships and building trust in order to help clients navigate systems for access to services.
- Provide advocacy, targeted case management and community based rehabilitation services to children and their families.Â
- Provide psycho-education about mental health issues and advocacy services for clients.
- Comply with policies, procedures, documentation and paperwork for EPSDT Medi-Cal billing and meeting billing requirements as set by the agency.
- Provide services in the community, for which travel by car is necessary.
- Provide stabilization support services to children and TAY in an in-person milieu environment.
- Excellent bilingual ability in English and Spanish
- Bachelor’s degree in Psychology, Criminal Justice, Social Work or related field (preferred)
- Direct, personal experience with foster care, social services, juvenile justice, mental health, residential treatment or group home placements, wraparound services, or other related community resources.
- Experience providing case management and rehabilitation services.
- At least 2 years experience working with system-involved youth.
- Demonstrated ability to work with families who have experienced high levels of trauma and demonstrate high mental health needs.
- Ability to work independently and collaboratively as part of a multidisciplinary team.
- Proven ability to be organized, manage time well, complete high quality and timely documentation.Â
- Clean CA DMV record and current car insurance and car; travel is required.
- Demonstrated experience working with children and families who may be involved in the child welfare system.
- Case management skill – assessment/evaluation, crisis intervention, ability to use supervision effectively, ethical and legal responsibilities, high quality documentation.
- Cultural humility - knowledge of issues of race, class and ethnicity.
- Efficiency and time management - ability to be well-organized and complete timely documentation.
- Collaboration and team functioning - able to work independently and collaboratively as part of a multidisciplinary team.
- Ability to work with clients who have complex traumatic histories and other attachment difficulties.
This is a full time, non-exempt position reporting to the Associate Director of Care Coordination. Salary range $67,000-$72,000 per year depending on degree level, inclusive of 10% Spanish language differential. Includes performance-based compensation plan, competitive benefits package, and 403(b) plan.
