WestCoast Children’s Clinic, located in Oakland, California, is a non-profit community psychology clinic that provides mental health services to Bay Area children, youth and families. The Bilingual (Spanish) Case Manager, as part of our Outpatient Mental Health Services, works within dynamic teams of youth, caregivers, mental health providers, intensive care coordinators, social workers, teachers and other supportive figures in a youth’s life. Services may include intensive skill building, advocacy, linking to and coordinating with tutoring & vocational programs, mentoring, shelter/housing, medical and mental health services, legal aid, education, access to benefits or other needs families may have. The Case Manager also provides a diversity of care through conducting short term needs and strengths assessments, providing Intensive Home Based Services (IHBS) and  stabilization support services to children and TAY in a drop-in center environment. An ideal candidate demonstrates flexibility, initiative, creativity, inclusivity and knowledge of the unique challenges youth and young adults face in foster care.

Our Outpatient Mental Health services are uniquely suited to meet the needs of vulnerable and high-risk children, adolescents and families – particularly those involved in the Alameda County foster care system.  We see clients at our Fruitvale clinic, via tele-health as well as in community settings, meeting families at their greatest point of need, equipping them with the tools and linkages to help overcome what often are multiple traumatic events. The impact of our work is evident in the lives we help to change on a daily basis. 

How you'll make an impact:

  • Work collaboratively as a treatment team member to support youth and families with developing and making progress toward their goals.
  • Manage a shared caseload and ensure 65% of work hours are dedicated to providing services to youth and families.  
  • Identify and link youth and families to services and resources 
  • Develop goals and provide individual rehabilitation; An intervention which focuses on skill building to improve life functioning. Examples include: Guidance on how to identify and apply to jobs or schools, build self care and hygiene skills, strengthening social skills, etc.
  • Support youth and families reduce mental health behaviors in the home and community through Intensive Home Based Services (IHBS) 
  • Conduct short term strengths and needs assessments to empower families in case planning.
  • Provide stabilization care to foster youth in a milieu environment as youth await and prepare for transition to foster placements.
  • Complete progress notes, assessments, and other clinical documentation which complies with policies and procedures for meeting EPSDT Medi-Cal billing  requirements as set by WCC
  • Meet with youth, families and community stakeholders in various community settings (homes, schools, shelters, parks, etc) to accommodate access to quality care. 
Qualifications:
  • Bilingual in Spanish and English (spoken & written) required.
  • Bachelor’s degree in Psychology, Criminal Justice, Social Work or related field preferred.  
  • 1 year of experience working with youth and families and/or providing case management, rehabilitation services or intensive home based services
  • Passionate about working with foster youth, families & young adults who have complex traumatic histories.
  • Highly Motivated to work from a team based approach. 
  • Values humility, compassion, self-care, and ongoing personal growth .
  • Knowledge of Alameda County resources for children and families preferred.
  • Ability to complete high quality progress notes and other clinical documentation; A writing sample exercise will be conducted during an interview.
  • Ability to travel by car to meet with clients, generally within a 90 mile radius of the WCC office.
  • Flexible schedule with ability to work evenings and occasional weekends as needed.
  • Clean CA DMV record and current car insurance and car; travel is required.

  • Understands the value of building healthy relationships to help youth navigate complex systems and how trauma can impact youth and team dynamics. 
  • Ability to use supervision effectively, uphold ethical and legal responsibilities, complete high quality documentation, assess needs, strengths & provide crisis intervention and de-escalation
  • Knowledge of or ability to learn Medi-Cal reimbursement procedures and billing.
Position details and compensation:

This is a full time, non-exempt position reporting to the Director of Care Coordination. Starting rate is 34.38 per hour ($71,500 per year), inclusive of 10% Spanish differential. Includes performance-based compensation plan, competitive benefits package, and 403(b) plan.
