of patients' health status and symptomsDevelop and implement individualized care plans in
collaboration with other healthcare providersManage medications and treatments to alleviate pain and
other distressing symptomsProvide emotional and psychological support to patients and their
familiesEducate patients and families about illness progression and available treatment
optionsCoordinate with social workers, chaplains, and other healthcare professionals to ensure
holistic careFacilitate advanced care planning and end-of-life discussionsMonitor and evaluate
patient progress, adjusting care plans as neededParticipate in interdisciplinary team meetings to
review and update patient care plansAdvocate for patients' needs and preferences within the
healthcare systemConduct and contribute to research in palliative care practices and
outcomesRequirements:Master's degree in nursing, with a specialization in palliative or hospice
careExperience in managing chronic pain and symptoms related to serious illnessesExperience working
within a multidisciplinary healthcare teamFamiliarity with electronic health records and palliative
care documentationTraining in advanced care planning and end-of-life care discussionsActive nurse
practitioner (NP) license and DEA for Washington stateMinimum of 2 years of experience in a
palliative care settingStrong clinical assessment and diagnostic skillsDemonstrated ability to
manage complex patient casesProficiency in patient education and counselingCommitment to continuous
professional developmentCompensation and Benefits:Our compensation is a productivity-based model
(Fee for Service) that offers you control to maximize your income potential. Expected compensation
is estimated salary range of $125,000 to $158,000 with no cap on productivity income potential. This
full-time role is benefit eligible.Full-time clinicians are eligible for an excellent comprehensive
