Mosaic Life Care, a physician-led, multi-specialty health-care system, is currently in search of a board certified/board eligible physician with an interest in Geriatric Medicine. This is a great opportunity for Family Medicine, Internal Medicine, or Palliative Care physicians with interest in a geriatric patient population to join our Care At Home program. The Care At Home program brings primary care as well as disease and symptom management to patients and their loved ones in their own home.

Practice Details:

• Provide care in a Home-based setting/Skilled Nursing Facility/Outpatient Clinic
• 24-hour Triage Nurse support for patients
• Monday to Friday schedule, daytime hours
• Call schedule is one day per week and every 6^th weekend
• System wide EPIC EMR
