Where you’ll see your impactÂ
As an exceptional Health Coach, you’ll be responsible for onboarding and supporting patients using the NuvoAir Home technology for remote (home) monitoring as well as coaching members to set and achieve health goals. This is very much a relationship-focused position designed to help our clinical partners and patients leverage our platform to its greatest extent. Our mission is to support people with long term respiratory conditions to engage in self monitoring of health, to understand their health data and to use data to have collaborative conversations with their healthcare teams. As a health coach with expertise in supporting behavior change, and a passion for digital health, you will be pivotal in helping our users to engage with their health data and use it to influence their health related behaviors.
What you’ll be responsible forÂ
Warm and Engaging We're looking for someone with a naturally warm and extroverted personality. You'll be connecting with older patients, requiring patience and the ability to establish a comfortable and trusting relationship.
Extensive Patient Interaction (90% of the time): You will spend at least 90% of your time actively engaging with patients through various modes of communication such as video calls, phone calls, SMS, and chat. You will work hours that align with your patient panel's location, with efforts made to match your time zone with that of your patients during the recruiting process.
Goal Setting and Follow-Up: You are responsible for setting up personalized health goals for each patient and ensuring these goals are met through regular follow-ups, encouragement, and adjustments as necessary.
Data Triage and Clinical Team Coordination: You will monitor patients' health status and are responsible for triaging important data to the clinical team, especially if a patient's health status shows signs of decline.
Additional Responsibilities:
Proactively reach out to patients to keep them motivated and engaged with our care model.
Onboard patients and provide regular check-ins, offering education on maintaining a healthy lifestyle with comorbidities.
Document updates and verify information in our EHR, ensuring the full clinical team has up-to-date health information.
Collaborate with the clinical team to discuss patient needs.
Assist patients with technical challenges related to NuvoAir technology.
Contribute to workflow and process improvements in collaboration with the Director and VP of Clinical Operations.
Healthcare experience as a background in the medical field is a must have
Exhibits a startup-ready mindset, adaptable to a dynamic environment where patient needs are prioritized.
Passionate about health and wellness, often engaging in conversations about healthy habits.
Comfortable in engaging conversations, and known for making others feel at ease.
Salary Range: $40-$45k/year USD plus bonus!!
Please note the national salary range listed in the job posting reflects the new hire salary range across levels and U.S. locations that would be applicable to the position. Final salary will be commensurate with the candidate’s final level and final location. Also, this range represents base salary only and does not include bonus, equity, or benefits if applicable.
