Description
Plans and coordinates activities related to transition planning. Develops and implements a plan of care designed to meet the patient's post hospitalization needs while ensuring continuity of care. Responsible for the collaboration with medical staff, patient, family, community agencies, or facilities involved in patient's care, and the health care team by assessing, facilitating, planning and advocating for health needs on an individual basis resulting in quality, cost-effective outcomes.

Required Qualifications:
- Master's Degree Social Work or Counseling
- Upon hire:
- Washington Clinical Independent Social Worker Associate License Or
- Washington Advanced Social Worker Associate License Or
- Washington Mental Health Counselor Associate License Or
- Washington Agency Affiliated Counselor Registration
- Experience in discharge planning, care coordination, or community resource navigation, in a clinical or community setting.
Preferred Qualifications:
- Experience in discharge planning for medical and or surgical patients in an inpatient or outpatient setting.
- Experience in an acute hospital setting doing discharge planning is preferred.

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
